Individual Supervision $75 hourly, an hour per 20 clients seen.
Group Supervision $50 an hour. Need an hour per every 20 clients.
No call/No show or a late cancellation: 50, except Medicaid and Medicare clients.
Cancel within 24 hours: $0
Children and Teens:
We require parents to be involved in intake, review of treatment plan, discussions on coping skills, barriers to coping and client’s overall progress. Parents are required to attend a session once every few months to review progress, coping, ect….
Court and Other Agency Involvement:
I do not attend court and if I do it is $300 dollars an hour and that includes prepping for court. If you have other agency involvement, please let me know as I may have to obtain a release of information to support you.
FMLA (FMLA will not be filled out until a clinician gets to know you. We will not fill out FMLA paperwork until we have met with you for at least 4 to 6 visits based on clinicians discretion. This allows us to build the knowledge to support the documentation you're asking for.
Scope Of Practice:
We are trained in providing services that we have knowledge and skill with. If we determine we are not the best fit, we will work with you in getting linked with another provider. We DO NOT prescribe medications. WE do not make recommendations for custody nor divorces. If there are children services case, custody issues and so forth, please let me know ahead time as our records most likely will be subpoenaed/court ordered.
Payment and Billing
You are given a bill and receipt after the session has been completed. The clinician will bill your card at that time and/or by the end of the day. UNLESS you have insurance. We will bill the copay if we know it and finish billing once the insurance company has reached out. We will not accept checks or cash at this time. You will be billed your copay initially. Please know what that is. We will complete necessary paperwork for your insurance but ask that you are aware that they can take weeks to pay and at times do not pay all of it. Once they pay I will charge your card the rest of the remaining balance. If you would like to be notified, please let us know.
Required Documentation for a Clinician:
A clinician will complete a Biopsychosocial Assessment, a mental status exam and measures for clients in the initial stages and ongoing. Every client will have a treatment plan that includes their goals, objectives in how to reach those goals and the interventions clinicians will utilize. This will be available to you and is a collaborative and ongoing live document. We will review your treatment plan every 90 days and document together what has changed and what we continue to work on. All documents are in your chart and we are required to keep your paperwork for 7 to 10 years. Information is stored in encrypted and locked EHR and encrypted hard drive. A clinician may ask for a release of information they can share via our EHR system so that we can communicate with your other providers.
Scheduling
Hello and Welcome to Ataraxia Wellness Project North, LLC. My names Amanda Price. When you have scheduled with us for your session, we will update the session in my EHR system Simple Practice. We will then send intake paperwork from Simple Practice to your email. Those documents give us permission to work with you as well as gather information that can guide us through our initial meeting with you. You will be able to use Simple Practice by downloading the app or going to a browser. You will make a username and password and be able to communicate via messenger to me. You can upload documents to your portal such as a previous assessment, fmla paperwork and/or other paperwork you would like to share with me.
Initial Session:
During our initial session we will be discussing what you want out of therapy and what therapy looks like with your clinician. We will complete an assessment to gather background information and measures to see the frequency, duration and severity of your symptoms. More often than not, sessions will start weekly and then decrease over time. We will explain confidentiality, mandated reporting and other necessary information in regards to obtaining therapy and what a clinician is required to do if needing to report something to the authorities.
